What happens next?
Once you have completed this form, a member of the Harassment and Misconduct Team will be in touch with you within 1 working day. Forms submitted by colleagues within the University and LUU will be managed by their relevant HR departments as soon as possible (but this may not be within 1 working day). Completing a form through Report & Support will not start
a formal complaints process. However, if you include identifying information such as someone's name, and we have concerns about risk, we may have to take action. Please read our
Confidentiality Agreement for further information.

What about my data?
Your data is important to us. The information you supply will be stored securely by the Culture Shift and the University of Leeds. Your anonymised data will be accessed by Harassment and Misconduct Team and key colleagues at the university appropriate to this line of work. We won't collect or store any information that goes against our
